At this time it is my opinion the forms provided by Cook, Lake, DeKalb and Madison counties to name a few are not compliant with the law and thus may prevent you from being able to recover damages against an employer who fails to withhold properly.

In re: Marriage of Smith - This is a case I won at the trial level in Winnebago County. The opposing party appealed and it was determined my client was entitled to the arrearage in excess of $60,000.00 by the Second District Appellate Court.
